Landlord Direct Deposit Authorization Form

I authorize Spartanburg Housing and the financial institution listed below to electronically credit my checking/savings account all Housing Assistance Payments (HAP) rent payable to me by Spartanburg Housing. 

I understand I have access to the Landlord Portal showing the amount and date of the HAP credited to my account. 

If funds that I am not entitled to are deposited in my account, I authorize Spartanburg Housing to direct the financial institution to return the funds. 

I understand if changes are made to my email address, mailing address, contact person information or bank account information, I must immediately submit those changes to cgodfrey@spartanburghousing.org before the next scheduled HAP payment. 

I understand a voided check or bank deposit verification form MUST be submitted to process the account addition/change.
